| Short-Term Disability |
- This plan provides you with income protection for illness or injury not related to work for up to 52 weeks either at 100% or 75% of base pay, dependent on years of service.

| Long-Term Disability |
- Provides a benefit equal to 70% of your monthly salary (to a max. monthly benefit of $16,000)
